Daveing without a Kippah

My friend asked me that he davened shachris with tefillin and a tallit from beginning until end and only afterwards realized that he was not wearing a kippah the whole time.

1) What is the status of his davening ? is it considered that he davened and is yotzeih (just he wasn't wearing a kippah, which seemingly is only a minhag)?

2) is his davening lacking in any way?

3) if it is the same scenario, but he has a kippa on, although he is in a makom metunaf (ie. bathroom) which is not fitting to pray, what is the status of that prayer? I assume here, it is as if he DID NOT daven as the place was not fitting.

but without a kippa in a fitting place for prayer, is ok?

Without kippah - he should have been more careful, but after the fact it's probably fine.

In the bathroom - he would probably have to repeat, although there is room to say that it's fine since our bathrooms are generally clean.
